Withdrawl Form

Instructions:

A 30-day notice is required by the 1st of the month prior to withdrawal. Mid-month withdrawals will not be prorated. Failure to provide proper notice will result in forfeiting your deposit. Your deposit will not be returned if your account is not current.

Example: Withdrawing on October 1st requires notice by September 1st.
Example: Withdrawing October 15th also requires notice to be given by September 1st. Since mid-month withdrawals are not accepted, your options are to pay the daily rate for the number of days your child/children attend the program, or pay October tuition's in full.
